Score a form
Give answers points, define outcome bands, and choose whether people see their result.
Before you start
- A form with at least one question.
Scoring turns answers into a result automatically. It's most common on audits and inspections, but you can use it on any form with questions.
Turn on scoring
- Open scoring On the Build tab, find Scoring (optional) and turn on Enable scoring for this form.
- Give answers points Select Manage scoring. For each question you want to score, set how many points each answer earns.
- Create outcome bands Add bands that turn the percentage score into a result. For example, 90% and above is Pass, 70–89% is Needs attention, and below 70% is Fail.
- Decide what people see Turn on Show the result to the person who submits if they should see their outcome straight away.
- Publish Scoring starts only when the form is published.
Important
Add a catch-all band that starts at 0%. Without one, low scores are calculated but get no outcome, and they're easy to miss in reports.
How scores are calculated
- Scores are calculated when a submission is submitted, not while it's a draft.
- Submissions are scored against the published revision. Changes in a draft have no effect until you publish.
- Submissions made before you turned on scoring stay unscored.
What team members see
If results are shown, the person who submits sees a message that matches their outcome. Depending on the result, it confirms the responses were recorded, asks them to review the flagged items with a supervisor, or tells them to check with a supervisor before starting work.
