Add content to a form
Questions, instructions, images, video, PDFs, file uploads, signatures and page breaks.
Before you start
- A form in draft. See Create a new form.
The Build tab has a palette of content blocks. Add blocks to the form, fill them in, and drag them into the order people should see them.
Content blocks
| Block | Use it to |

| Text / description | Give instructions, safety notes or context. Supports basic formatting. |
| Question | Ask something people must answer. See Question types. |
| Image | Show a reference photo or diagram (JPG, PNG or GIF). |
| Video | Show a short how-to clip (MP4). |
| PDF file | Attach a procedure, manual page or policy for people to read. |
| User file upload | Ask people to attach a photo or document (up to 10 MB per file). |
| Signature | Capture a signature on the device's screen. |
| Page break | Split a long form into pages so it's easier to work through on a phone. |
| Stop check | Pause the form until a reviewer approves. See Add a stop check. |
Add and arrange blocks
- Add a block Choose a block from the palette. It's added to the form.
- Fill it in Type the question or text, upload the media, and set options such as Required.
- Reorder Drag blocks to change their order.
- Preview Select Preview to see the form as people will see it.
- Publish when ready Changes stay in the draft until you publish. See Publish a form and manage revisions.
Tip
Put a short text block at the top saying how long the form takes and what people need before they start, such as the asset tag number or the customer's name.
Note
Keep each page to a screen or two on a phone. Use page breaks between natural stages, such as Before starting, During the work and Sign-off.
